Arizona Attorney General’s Office Offers Free Life Care Planning Packets
The Arizona Attorney General Office (AGO) offers Life Care Planning packets to the public — an important free resource. The documents include all of the necessary forms to help Arizonans make their choices known about who will and how to manage their medical affairs in the event of an emergency.
They include:
- Health Care Power of Attorney
- Living Will
- Mental Health Care Power of Attorney
- Pre-Hospital Medical Directive (Do Not Resuscitate)
The forms should be signed, witnessed and registered with a third party agent of the state so that health care providers can access them in an emergency.
“The Attorney General’s Office wanted to make sure that people had free and easy access to these really important medical health care power of attorney documents. We never know what could happen to us, so it’s really important that everyone fill out these forms to make sure that their wishes are honored,” said Courtney Bennett, the AGO’s Director of Community Outreach.
The AGO provides free Life Care Planning packets to Arizonans to list medical wishes. You can request a life care planning packet through the mail or download one.
